Junior Counseling Forms
How do families share important information with Counseling Services regarding the welfare of their son/daughter?
Families are required to complete a Counseling Services form prior to a student enrolling at NCSSM. The form is submitted electronically on Blackbaud, but it is also available for families to complete via a printed copy.
Families are provided the opportunity to provide information about a student’s social, psychological, and emotional history. Counseling Services often follows up with the family prior to the student coming to campus regarding the responses to the form. Responses are read by the counseling staff although specific information may be shared with individual NCSSM professional staff on an as needed-basis. It is required that parents contact either the Dean of Counseling Services or the assigned counselor if any of the information changes while a student is at NCSSM.
The form is separate from the official permanent record and is a confidential record kept in the Counseling Services Office until graduation.
